Typically, coaches handle signature requests because then they know what events their kids are signing up for and they can request a group of skaters' signatures at one time.  Depending on the form, it either asks for a club officer or requires the signature of President, Vice President, or Secretary.  PLEASE do not wait until the day a form is due to acquire a signature!  If it is a true "deadline situation", send it in without a signature but photocopy the signature page, get the photocopy signed, and mail it ASAP.  This is better than having the whole application submitted late.  Skaters should always photocopy their entry forms before sending them in case there are questions later.

The person needing the form signed can either see one of the officers at the rink in person, email them, phone them, or in some cases fax them.  If you need to fax a form to be signed, always contact the officer first to make sure the officer is able to accept faxes and is at home!
